Care questionnaire
A short, friendly questionnaire about daily needs and preferences.

Assessment visit
We meet with your loved one to complete the LIC 603 Preplacement Appraisal.

Physician's assessment
Their doctor completes the LIC 602A Physician's Report and a TB test.

Review the admission agreement
Read every page — it's posted on our Documents page — and ask anything.

Move-in day
We welcome them home and create a personal care plan within 2 weeks.
Move-in checklist
What to bring — and what to leave home
Bring
- Comfortable clothes and walking shoes, labeled
- Favorite chair, bedding, or quilt
- Family photos and keepsakes
- Toiletries and personal care items
- Glasses, hearing aids, dentures, and chargers
- Current medications in original containers
- A small TV or radio, if desired
Leave home
- Valuable jewelry and large amounts of cash
- Space heaters and extension cords
- Area rugs that could cause trips
- Large furniture that crowds walkways
- Unapproved over-the-counter medications
Paperwork
Documents you'll need
- Photo ID and insurance cards (Medicare, supplemental, etc.)
- Physician's Report (LIC 602A) and TB test results
- Current medication list from the physician
- Power of attorney / advance directive, if any
- Emergency contacts
- Signed admission agreement
