Levels of care

Care in plain language

A little help

Mostly independent. Reminders for medications, a hand with bathing, and someone nearby day and night.

Regular help

Daily hands-on help with dressing, bathing, toileting, or getting around with a walker or wheelchair.

Extensive help

Help with most daily activities, including residents who are non-ambulatory and need assistance moving.

Memory & dementia support

Familiar routines, familiar faces, and meaningful activity — garden time, music, simple chores, and walks on a safe, secure Commons. Our caregivers complete state-required dementia care training, and we adapt each day to the person, not the diagnosis.

Hospice support

We're applying for a hospice waiver (pending) so residents can stay in the home they know at the end of life, with care from a licensed hospice agency and comfort from the people who know them best.

Honesty first

What we can and can't provide

We can

  • Accept non-ambulatory residents
  • Support residents living with dementia
  • Help with medications as allowed by state rules
  • Welcome visiting home health, therapy, and hospice providers

We can't

  • Provide skilled nursing or medical treatment — we're not a nursing facility
  • Accept residents whose primary diagnosis is a mental-health disorder unrelated to dementia
  • Care for needs that require around-the-clock licensed nursing

If we're not the right fit, we'll tell you — and help point you to resources that are.

Come see the Commons for yourself.

Walk the trail, meet the team, and stay for lunch. Tours are relaxed, honest, and never high-pressure.
